Application and Pre-Screening Process

Our application and pre-screening process is extensive. This allows us to ensure we are finding the right donor candidates for our program. The process of egg donation is not simple yet it can be extremely rewarding for all involved. While the entire process usually takes 3-5 months from start to finish, most of your participation takes place within the last two-three weeks. The process is described below. Rest assured we will be here for you each and every step of the way!

Registration

The first step in becoming a donor for Special Creations is completing the Donor Registration Form. Once this initial application has been approved, you will receive a username and password to access the complete donor application. If you meet the initial qualifications we will have you complete our donor application and submit photos of yourself and/or your children. A medical release form will also need to be completed so that our Medical Director can retrieve and review your medical records from your OB/GYN and/or primary care physician from the past 3-5 years.

Application

Once you receive your username and password, log-in to your account with Special Creations and complete the full application. You may save your progress as you go and come back at a later time until the full application is complete. If it is determined that you are a good candidate at this point you will be required to sign a contract in order to continue further screening.

Medical Records

You will need to complete a Medical Records Release Form which will allow Special Creations to retrieve your OB/GYN records from the last 5 years. This information is reviewed by our medical director as part of the screening process and will be held confidentially according to the rules and regulations of HIPPA.

Photos

4 photos are required as part of the application process. One each for the following categories: infant, young adult, and two current photos as an adult (one upclose and one full length). The photos can be uploaded on our website or mailed. If you decide to mail your photos we will return them to you.

Contract

Once our Medical Director has screened and approved the application and Medical Records, a contract will be offered and must be signed prior to continuing the screening process. Once your contract is signed you can begin the pre-screening process. This process consists of the following:

  1. Hormone level screening
  2. Drug screening
  3. Pelvic Exam/ Ultrasound
  4. Infectious Diseases screening
  5. Psychological Testing
  6. Genetic diseases screening

The screening process is completed over approximately 10 days and 3-4 visits. Once you are cleared through this process you are ready to be listed on our donor panel. Because our screening process is so thorough and completed in advance, you will have an excellent chance of being selected by a recipient.

Labs/Exam

After the contract has been executed, blood tests, urinalysis, and a pelvic exam will be required. These tests screen for sexually transmitted diseases, baseline hormone levels, common genetic disorders, and drug use and will be performed under the direction of our medical director.

Psychological Evaluation

The final step is a psychological evaluation performed by a licensed therapist. as required by the Food and Drug Administration for egg donors. This evaluation includes the MMPI-2 and a personal interview.

Upon completion of this process, you will be accepted onto our donor panel.
